Currently a soon to graduate medical student wanting to getting a custom painting of my mentor operating. Unfortunately, it's hard to get a perfect picture of them, but I have close-ish ones of them operating and then photos of them normally+a photo for inspiration of what I am looking for. Would it be possible to synthesize all this into a single image? If so I'd be interested in ordering a few for my mentors. Thank you!

Yes. You can supply reference photos of the other people you want to add if you also provide detailed instructions. Keep in mind how they can be added and how natural it will end up looking depend on the quality and orientation/angles of the other photo you are trying to add.
